To save time when visiting our office, please complete our new patient paperwork before your scheduled appointment.

Our patient forms can be downloaded to your computer, completed, and submitted to our office in one of the following ways:

• Email your completed paperwork to our office (info@amatxmed.com).
• Print and mail your completed paperwork to our office mailing address.
• Print your completed paperwork and bring it with you to your first office appointment.

Please be prepared to provide this new patient information to our office along with your current medical insurance information. If you have any questions when filling out these forms, please do the best you can and our staff will assist you on the day of your office visit.
